New student enrollments are done through appointments only. Please contact our office (386-322-6100 extension 38497) to set up an appointment to meet with our registrar or if you have any questions concerning the enrollment process. Enrollment appointments are held daily from 7:30 AM to 10:30 AM. Students will meet with their school counselor the next morning following their enrollment appointment with our registrar.

The following items are required for ALL students:

1. Student Entry Form - all gray areas must be completed including the Home Language Survey.

2. Telecommunications Policy - Internet access permission form

3. Request for student records

4. Code of conduct policy and procedures notice

5. Emergency card

First Time Florida Enrollment:

1. Two proofs of residential address (Gas, Lights, Water, Rental Lease, Mortgage)

2. Current physical (must have been completed within the last 12 months)- done in the state of Florida

3. Immunization records - Florida Form 680. All students must have Hepatitis B series, TB, and second MMR.

(We are unable to enroll anyone without a current FLORIDA IMMUNIZATION RECORD/SHOT RECORD.)

4. Birth certificate

5. Social Security Number

6. If available, a copy of a transcript or last report card (if not available, please bring name, address and phone number of last

school attended.

Florida In-State Transfer:

1. Two proofs of residential address (Gas, Lights, Water, Rental Lease, Mortgage)

2. Records from previous school showing courses, credits earned and immunization records

3. Birth certificate

4. Social Security Number

In-District Transfer:

1. Two proofs of residential address (Gas, Lights, Water, Rental Lease, Mortgage)

2. Complete In-District Transfer form

3. Withdrawal Form from previous school
